Testing for Female Infertility: What to Expect

Comprehensive Screening for Female Infertility

When visiting a fertility specialist, comprehensive screenings are conducted to determine the most accurate findings and identify appropriate treatments to enhance conception. During your initial visit, expect a thorough review of your medical history, a physical examination, and blood and urine tests to screen for sexually transmitted infections (STIs). Additional tests may include:

  • A hormonal evaluation assesses ovulation and hormonal imbalances, including those related to reproductive hormones such as estrogen. Additionally, a thyroid function test may be conducted to determine if there is an overproduction or underproduction of thyroid hormone, which can impact fertility.
  • A pap smear is performed to check the health of the cervix. During this procedure, cervical cells are collected and sent to a lab for analysis, which can help detect cancer or other suspicious changes.
  • A basal body temperature (BBT) test involves measuring your morning body temperature while still in bed, using an ultra-sensitive thermometer. This test is based on the fact that your ovaries release the hormone progesterone during ovulation, causing a slight increase in temperature that remains until menstruation when it decreases. Any disruptions in this pattern may signal a problem with ovulation.
  • An endometrial biopsy is a minimally invasive procedure performed when basal BBT results are unclear. During this procedure, a small sample of endometrial tissue is removed and examined under a microscope. This test assesses the endometrium (the lining of the uterus) for changes that indicate ovulation has occurred and evaluates the endometrium's receptivity to a fertilized egg. The biopsy can help determine if the endometrium has undergone the necessary hormonal changes to support a pregnancy.
  • Ultrasound imaging (sonography) uses sound waves to locate blockages, structural issues, or diseases affecting the fallopian tubes, ovaries, and uterus. A saline infusion sonogram (sonohysterogram) may also be performed to provide an internal view of the uterus.
  • A post-coital test determines if sperm can survive in cervical mucus and reach the uterus for fertilization.
  • A hysterosalpingogram (HSG) is an X-ray procedure in which contrast dye is injected into the uterine cavity to detect structural issues.
  • Laparoscopy may be performed to examine the pelvic organs, such as the uterus, fallopian tubes, and ovaries, for any abnormalities or blockages that may be contributing to infertility.
